Diabetes (Community)

The aim of the Diabetes Service is to provide education and professional support in the management of diabetes to individuals with diabetes, their carers and healthcare professionals.

The service is delivered within a variety of settings to ensure easy access i.e. GP practices, diabetes centre, patients homes, health centres. The team includes diabetic specialist nurses for adults and children and an extended scope podiatrist.

The service provides advice to enable people with diabetes to achieve and maintain blood glucose as near normal limits as possible and minimise the risk of diabetes associated risks to health.

Office: Monday to Thursday 8.30am – 4.30pm, Friday 8.30am – 3.30pm
Clinical hours: Monday to Friday 9am – 5pm

The service does not operate on bank holidays
