What is “My PreOp” questionnaire?

  • You will have been sent a link to complete your My PreOp questionnaire prior to your clinical appointment.
  • It is essential that you compete this prior to your Preassessment otherwise this could delay your planned surgery.
  • The process on average 15-60mins dependant on your past medical history

Why do I need to complete the “My PreOp questionnaire”?

The My Preop questionnaire is a digital pre-appointment screening tool that allows us as Clinicians to see all of your relevant medical information to inform any suggested tests or actions may need to be taken. It also allows us to review your information in preparation for your Preoperative assessment appointment to see whether you will be passed fit for surgery.

Do you need support to complete your My PreOp questionnaire?

Should you have any issues or difficulties in completing the My PreOp questionnaire, help is available at our “Walsall Connected Healthcare Island.” We have staff available to hand from 8am – 4pm, Monday to Friday, this is located on Route 126 in the middle of atrium. You can contact the hub using the following telephone number: 01922 444040 Ext. 7009.

What do I do if I need an interpreter?

You are entitled to bring along a family relative or friend with you to interpret for you during your Preop assessment appointment prior to surgery. Should you require it, you can access the 360-interpretation service that we provide, and we cater to all languages. Therefore, if your English is limited or you need the services of a sign language interpreter, please contact the Pre-operative Assessment Clinic as soon as possible so that an interpreter can be arranged.

Changing or cancelling your appointment in advance

This appointment is integral in your preparation for your surgery. Should you be unable to attend your appointment, please let us know as soon as possible, so we can rebook your appointment. You can do this by calling the number on your letter. Alternatively, you can contact the Access team, by calling 01922 444000, Lines are open 8am – 6pm, Monday to Friday (excluding Bank Holidays).
