Community Nursing

The Community Nursing Service covers all residents in Walsall who are registered with a Walsall GP.

The following services are provided:

  • Co-ordinated care provision and case management for patients with long-term conditions

  • Planned programmes of nursing intervention and supportive care

  • Referral and liaison with other agencies to promote maximum independence

  • Advice and information on a range of health issues.

The service provides nursing care predominantly in patients own homes. Nurses will visit non-housebound patients whose nursing needs indicate it is more appropriate for them to be seen in their own homes.

Some care considered to require the specialist skills of the District Nursing Service such as wound care, are delivered in clinic settings as close to the patients home as possible.

The service operates 24 hours a day, 7 days a week.

Clinics are based across Walsall.

These teams are available Monday to Friday 8am to 6pm. A patient’s first point of call between 8am and 6pm should be their relevant hub number. Urgent calls between the hours of 6pm and 10pm Monday to Friday are covered by 01922 604920.  Between 10pm and 8am and weekends please contact the Manor Hospital Switchboard on 01922 721172.
