Chronic Obstructive Pulmonary Disease (COPD)
The Chronic Obstructive Pulmonary Disease team is made up of specialist nurses who support patients with COPD in the community.
The service provides disease management support, encourages self-management and supports patients through exacerbations with a view to avoiding hospital admission where it is safe to do so.
Opening times are Monday to Friday, 8.30am – 4.30pm
