Community Specialist Palliative Care Nursing Team

What we do  

The Community Specialist Palliative Care Nursing Team delivers specialist palliative care to patients with a palliative care diagnosis or a life limiting condition living within a community setting.   We offer advice on symptom control, emotional support, information and explanation about patient's condition.

We provide Specialist Care that encompasses the holistic needs of our patients including physical, psychological, social, spiritual and financial needs in partnership with GPs, Community Nursing Teams, local hospices, hospital teams, palliative therapies team, bereavement services, complementary therapies team, clinical psychology, benefits agency and information and support services.

We may also intervene earlier in a patient's care to advise on specific symptom control issues.

Referrals are accepted from professionals currently involved in the care of the patient and contact with the service may vary from a single assessment visit to a period of on going support.
