Going home from the Neonatal Unit is a happy time. However, the thought of being at home without any support can be frightening, we have a Community
Outreach Service. The service aims to ease the transition from hospital to home.
Support is provided after going home by a trained neonatal nurse. This should help you and your family to enjoy being at home together with your baby.
During the visits from the Outreach Nurse you can expect your baby to be weighed and his/her growth to be monitored. If you are worried or concerned you can ask for advice. The Outreach Nurse can contact the paediatric doctors and arrange an extra checkup if necessary.
When the nurse feels that it is time, she will contact your health visitor and handover baby's care.
