What we do
The Community Nursing Service provides:
- Co-ordinated care provision and case management for patients with long term conditions
- Planned programmes of nursing intervention and supportive care
- Referral and liaison with other agencies to promote maximum independence
- Advice and information on a range of health issues.
The service provides nursing care predominantly in patients own homes. Nurses will visit non housebound patients whose nursing needs indicate it is more appropriate for them to be seen in their own homes.
Some care considered to require the specialist skills of the District Nursing Service such as wound care, are delivered in clinic settings as close to the patients home as possible.
Hours of Operation
24 hours (7 days a week)
Covers all residents in Walsall registered with a Walsall GP.

These teams are available Monday to Friday 8:00 am to 6:00 pm. A patient's first point of call between 8:00 am and 6:00 pm should be their relevant hub number.
Urgent calls between the hours of 6:00 pm and 10:00 pm Monday to Friday are covered by 0845 113 0761. Between 10:00 pm and 8:00 am and weekends please contact the Manor Hospital Switchboard on 01922 721 172.
