Chronic Obstructive Pulmonary Disease (COPD)

What we do

The COPD Team are a multi disciplinary group of specialist doctors, nurses and physiotherapist caring for patients in the community who have Chronic Obstructive Pulmonary Disease (COPD). They run a two-tier service which involves community clinics and exacerbation management with the aim of avoiding admission to hospital wherever it is safe to do so.

Clinics are currently run by the Lead Nurse from the Palliative Care Centre and aim to help with the management of patients with COPD. The nurse-led admission avoidance service aims to treat exacerbations in the home according to national guidelines and empower patients to manage their disease effectively, therefore reducing unnecessary admission to hospital.

Hours of Operation 

Monday- Friday, 8.30am - 4.30pm. Out of hours cover provided by Rapid Response.
